The Fertility Center is committed to providing you with the best possible care at a reasonable cost. If you have medical insurance, we will help you receive your maximum allowable benefits.
If you have an insurance plan that our office participates with, we will verify your benefits and file your claims for you. All copays, deductibles, coinsurance and any non-covered services are the patient's responsibility. If your plan has a copay for office visits, please be prepared to pay this at the time of service. We may collect this prior to you being seen by a provider. If your insurance requires a referral, you must obtain a referral from your primary care physician prior to your visit. If you do not have a referral, you will be asked to either reschedule your visit or sign a waiver and pay in full for all services rendered at that appointment.
If you have an insurance plan that our office does not participate with, we will give you the proper receipts and claim forms for you to file your claims. Payment for all services will be due at the time of your visit.
We encourage all patients, whether we participate with your insurance or not, to confirm your infertility benefits with your insurance company prior to visiting our office. Most employer groups have certain infertility benefits which may differ from that of another employer group, so it will benefit you if you check into your benefits before you come for your appointment.
